The Parkinson’s Passport enables you to complete an information booklet about your medications and treatment and then carry it when you are out and about or travelling abroad. This passport has been created thanks to the active involvement of the EPDA membership, healthcare professionals and our industry partners.

Instructions for use

There are several versions of the passport so please select the one appropriate to your treatment.

  • General - this version includes pages for you to complete detailing your medication dosage and timing. All of this information is also included in the versions listed below which also include additional treatment pages
  • Apomorphine
  • Duodopa
  • Medtronic deep brain stimulation
  • Vercise deep brain stimulation

Before you print out your Passport, you can personalise it by clicking onto the pages and completing the information that you wish to include. You can also print off additional medication sheets if required.

This document is for you to use as you need. You may want to take it to your doctor so he/she can help you fill in the details if you are unsure.

In addition to the above, our Parkinson's Passport has been translated into Arabic by Dr Walaa Ahmed Kamel and Dr Jasem Al-Hashel (movement disorder specialists, Kuwait). The EPDA accepts no responsibility for any errors or omissions in this translated version.
