The Parkinson’s Passport is designed so that you can complete information about your Parkinson’s disease medication and treatment and carry this with you when you are out and about or travelling abroad.
If a situation arises where you need to describe your condition and the medication you require to someone who may be unfamiliar with Parkinson’s, this document will help you to communicate effectively so that they can understand and help you to receive the correct and proper treatment.
INSTRUCTIONS FOR USE
Please select from the available languages the Parkinson’s Passport that you wish to view.
Before you print out the Parkinson’s Passport, you can personalise the document by clicking onto the pages and completing the information that you want to include.
You do not have to print the entire document as some pages may not apply to you. Likewise, you may want to print out a particular page more than once, for instance additional medication sheets.
