The EPDA PD Doc

Background

In 1995, the EPDA, with F.Hoffmann-La Roche Ltd, produced the flexicard.  The size of a credit card it was designed so that people could keep it in their wallets or purse along with their credit cards

The card had space for the person to write the name, address and telephone number of an emergency contact in the event that help was needed.

At the back of each card is written:

I have Parkinson's disease. Please allow me time.
    In case of an emergency contact ......


Each card contained this statement in 5 other languages including English. In all, 3 cards were produced that covered 12 languages. 

However, it was clear from the many requests the EPDA received, people wanted a card that covered many more languages.

The PD Doc

The PD Doc design allows users to choose the languages they want to appear on their document.  It also allows users to directly personalise the information online before printing.

  • Available online, the PD Doc is accessible to anyone who has access to the Internet and a printer
  • It allows anyone to choose which languages they want to include
  • Personal details can be entered directly onto the document before printing
  • The PD Doc can be easily completed and carried around either in a pocket, wallet or purse.
  • If you lose or damage the PD Doc, just go online and print another.
